Enhanced Service "eBaby" Available Again on Diia Portal
The Diia portal has reintroduced the improved service "eBaby", allowing for the quick submission of up to 9 government services through a single application.
Consequently, the Odessa City Council’s Administrative Services Center is also resuming the acceptance of documents for this electronic public service.
Parents can submit an application for the "eBaby" service if:
- the child does not yet have a birth certificate;
- the child was born in a medical facility in Ukraine;
- the parents are citizens of Ukraine.
To submit the application, you will need:
- a marriage certificate (if available),
- passports of both parents,
- TIN (taxpayer identification number) of both parents,
- UDR (unique registration number in the Unified State Demographic Register) of at least one parent,
- electronic medical birth certificate or medical birth certificate (form No. 103/o),
- a certificate of refusal from the one-time natural assistance "Baby Package" from the maternity hospital,
- the social bank account number of the recipient of child benefits (the parent on parental leave),
- a "PrivatBank" card,
- a special account for the "Baby Package" (in case of refusal from the natural assistance).
To access the comprehensive service, parents of newborns must register in the electronic queue or call 705-55-55, selecting a convenient time for the appointment.
